Lost your ID? You can request up to 2 free replacements online at madisonhs.org under Student Life. Any additional replacement requests require a donation of any amount to James Madison High School prior to issuance. Donations may be submitted online via Student Life > Webstore > Donation.
Please fill out this form to request a new ID. Your ID will be ready the next school day if submitted before 2pm. Any request received after 2pm will take an extra school day. Your new ID will be at the table by the main scanning entrance the following morning.
